Reminders:

If you are married, you must have the withdrawal form notarized.

Documentation must always be attached to the withdrawal form for hardship withdrawals.

How do I calculate the gross amount? Divide the Net Amount by 1 - Tax. For example, if you want to withdraw a net amount of $2000 with 20% tax withheld. Then the gross amount will be $2000 divided by .8 (1 - .20). The gross amount will be $2500.
