How To Use Reporting Forms
It is important to have the proper information before you report benefits. There are several different contracts based upon the type of carpenter hired, area worked and/or employer's membership in a collective bargaining agreement. Below are the basic steps to fill out and report carpenter benefits.

- Carpenters’ Combined Funds, Inc. (CCF) is the third administrator that collects fringe benefits for the Greater PA RDC. Contact this office, if you need reporting forms or have a question about fringes.
- Contact The Greater Pennsylvania Regional Council of Carpenters (GPRCC) if you have questions about wage rates or employment verification.
- Required Documents to report benefits
- Signed Contract from GPRCC
- Settlement Sheet – reporting form for submitting fringe benefits.
- Continuation Sheet – reporting form used to list employees, wages and hours.
- Filling out Reports
- Determine the contract the carpenters worked under – if you are unsure which report to use or do not have the proper form; please call CCF to send the proper forms.
- List employees’ payroll information on the continuation sheet for the period being reported.
- Total the hours worked, hours paid and wages columns.
- If the contract is based upon only hours worked, hours paid does not need to be calculated or vice versa
- The working dues not equal to 3% (contracted rate for majority of contracts) column only need to be filled out if an employee did not have the contracted rate withheld from his/her pay.
- Credit union should not be submitted to CCF. This employee deduction must be remitted directly to the Credit Union. If credit union is submitted to CCF, we will refund this money back to the employer.
- Transfer totals from continuation sheet to total fields on the settlement sheet.
- Calculate each individual fund accordingly.
- Total settlement report, sign and date report.
- Cut your check for the total of all reports being reported.
