HIPAA is the acronym for the Health Insurance Portability and Accountability Act of 1996. HIPAA provides rights and protections for participants and beneficiaries in group health plans.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
- Personal Representative Assignment
This form allows others access to your protected health information or be granted to take any action on your behalf.
- Opt-Out Form
This form is used to prohibit a deemed personal representative access to protected health information.
Purpose of HIPAA:
The Greater Pennsylvania Carpenters Medical is required by law to take reasonable steps to ensure the privacy of your personally indentifable health information.
