Below are the applications and forms needed to withdrawal funds from the Annuity and Savings Fund. Please read the application documents fully and call the Fund Office with any questions you may have.
Withdrawal forms are based upon a member's status. Please select the following forms:
Hardship Withdrawal Form for ACTIVE Members
Reminders:
- If you are married, you must have the withdrawal form notarized.
- Documentation must always be attached to the withdrawal form for hardship withdrawals.
- How do I calculate the gross amount? Divide the Net Amount by 1 - Tax. For example, if you want to withdraw a net amount of $2000 with 20% tax withheld. Then the gross amount will be $2000 divided by .8 (1 - .20). The gross amount will be $2500.
